Creatine for Healthy Aging: Muscle, Strength and Brain Energy

Creatine monohydrate is one of the most extensively studied nutritional compounds in exercise and performance medicine, but its relevance extends well beyond athletes. Creatine helps cells rapidly regenerate ATP, making it especially important in tissues with high and fluctuating energy demands, including skeletal muscle and the brain.

For healthy aging, the strongest evidence is in muscle. In older adults, creatine combined with resistance training can improve strength and lean tissue outcomes beyond training alone. What Creatine Actually Does

Creatine functions primarily through the creatine-phosphocreatine energy system.

Cells use ATP, or adenosine triphosphate, to perform work. During activities that require rapid energy, ATP is consumed quickly. Phosphocreatine helps donate a phosphate group to ADP so that ATP can be regenerated rapidly.

This system is particularly important during resistance training, sprinting and other short bursts of intense muscular activity.

The same basic chemistry also operates in the brain, where neurons must continually manage changing energy demands.

Creatine therefore acts less like a stimulant and more like an intracellular energy buffer. It helps tissues maintain access to rapidly usable energy when demand increases.


Creatine, Muscle and Healthy Aging

The muscle research is where creatine earns its strongest place in longevity medicine.

Resistance training remains the foundation. Creatine does not replace lifting weights, adequate protein or sufficient calories. When creatine is added to resistance training, however, studies in older adults have found additional improvements in lean tissue and strength.*

Creatine and the Brain

A 2026 systematic review of studies in adults age 55 and older found that most of the included studies reported a positive association between creatine and cognition, particularly memory and attention. The authors also emphasized that the evidence base remains limited and that better controlled clinical trials are needed.

Other reviews have found more mixed results. Creatine clearly can influence brain creatine availability, but higher brain creatine does not automatically translate into measurable cognitive improvement for everyone.

There may ultimately be groups who benefit more than others, including people with lower dietary creatine intake, older adults or people experiencing unusually high metabolic or cognitive demand. Research is still working through those questions.

For now, we consider brain support an intriguing additional reason to study and use creatine, while the muscle and exercise evidence remains the more established foundation.


Creatine, Protein and Hormone Health

Muscle preservation rarely comes down to one intervention.

Creatine works best inside a physiology that is actually being asked to maintain muscle.

That means progressive resistance training. It means adequate dietary protein. It means enough total nutrition to support recovery. Sleep matters. Metabolic health matters. Hormones can matter considerably.

Estrogen, testosterone, thyroid hormones, growth hormone and IGF-1 are among the hormonal signals that influence muscle physiology, protein turnover, recovery and body composition. Hormone therapy is not appropriate for everyone, and creatine does not correct a hormonal deficiency. They address different pieces of the physiology.
