How Much Longevity Testing Do You Really Need? Function Health, Fountain Life and Physician-Guided Care

Physician and patient reviewing laboratory results, cardiovascular imaging, DEXA and cognitive health data during a longevity medicine consultation.
One-Minute Read:

Longevity medicine now includes several very different models. Function Health® provides broad laboratory testing and longitudinal data at an unusually accessible price. Fountain Life™ combines laboratory testing with advanced imaging, genomics and physician review through a much larger early-detection program. HormoneSynergy® uses testing within a physician-led medical practice, with focused diagnostic services as well as a comprehensive Optimal Aging Assessment.

Each approach can be useful. The best choice depends on whether someone primarily wants more laboratory data, an extensive search for otherwise silent disease, or a physician who can interpret findings in the context of symptoms, medical history, hormones, cardiovascular risk, body composition, bone health and cognition.

Testing earns its place when the result can clarify risk, change treatment or guide surveillance. It becomes less useful when an increasingly large report produces incidental findings without a clear plan for deciding what deserves attention. Cost also extends beyond the initial package. Additional imaging, prescriptions, specialist care and treatment may not be included. A fair comparison therefore asks what is included at the published price, who is responsible for medical decisions and what happens after the results arrive.

We are seeing more patients arrive with extensive outside testing already completed. Some bring laboratory reports from Function Health®. Others have completed imaging and diagnostic testing through Fountain Life™. Much of this information is thoughtful and clinically useful.

It also raises a practical question: how much testing does a healthy or relatively healthy person actually need?

A 48-year-old woman entering menopause with rising cholesterol and a family history of osteoporosis has different needs from a 62-year-old man with an elevated coronary calcium score. Someone with substantial hereditary cancer risk deserves a different discussion from someone primarily concerned about maintaining muscle with age. The best evaluation provides enough information to understand meaningful risk and make better decisions, even when it is not the evaluation that produces the most data.

Three Approaches to Preventive Health at a Glance

Function Health®, Fountain Life™ and HormoneSynergy® are not equivalent services offered at different prices. They are built around different ideas of what preventive health care should provide.

Function Health®: Broad Access to Laboratory Data

Function Health® has addressed a real limitation in conventional health care. Many useful laboratory tests have historically been difficult or expensive for patients to obtain. Its current $365 annual membership provides more than 160 laboratory tests over the course of the year, including advanced cardiovascular, metabolic, hormonal, thyroid, inflammatory and nutritional markers.

The results are organized longitudinally and presented with educational interpretation. An ApoB result becomes more useful when a patient can see where it was two years ago. The same applies to fasting insulin, A1C, ferritin, thyroid function and other measurements that change with age, weight, medication and lifestyle.

Function Health® has also expanded into separately available MRI, CT imaging and other specialized testing. For someone who already has an established physician willing to review the findings and act when appropriate, the platform offers an unusually broad amount of laboratory surveillance for the price.

The limitation is not the quality of the data. A laboratory platform does not replace the physician who must decide which abnormalities carry meaningful risk, what requires treatment and what can simply be observed.

Fountain Life™: A Broader Early-Detection Program

Fountain Life™ approaches longevity through advanced imaging, laboratory testing, genomics and other technologies intended to identify disease or meaningful risk before symptoms develop.

Its current CORE program is priced at $10,500 per year. Fountain Life™ currently describes CORE as covering 8 of 27 listed diagnostics. Those covered diagnostics include full-body and brain MRI with AI, DEXA, more than 100 blood biomarkers, coronary CT angiography with AI, ECG, low-dose lung CT, whole-genome sequencing and blood-pressure assessment. The program also includes physician interpretation, an optimization plan and access to its Zori AI medical platform.

The APEX membership is currently listed at $21,500 per year and expands the diagnostic and ongoing-care structure. Other tests, treatments and services remain available à la carte. The published membership price should therefore be separated from the total cost of any additional testing or treatment a patient ultimately chooses or requires.

For someone with the financial means who wants a broad search for otherwise silent cardiovascular disease, cancer or structural abnormalities, the model has clear appeal. We have reviewed Fountain Life™ results brought into our practice, and much of the information is thorough and useful. Its cardiovascular imaging, DEXA and established metabolic testing can be particularly valuable.

The Tradeoff of Broader Screening

The strongest argument for broad screening is the possibility of finding a serious condition early. A renal mass, substantial coronary plaque, osteoporosis or another important abnormality discovered before symptoms appear can change care. Broad imaging also identifies incidental abnormalities. Many prove harmless, while others lead to repeat scans, specialist consultations or biopsies before their importance is known.

The American College of Radiology has not recommended routine total-body MRI screening for asymptomatic people without relevant risk factors or family history, citing insufficient evidence of benefit and concern about unnecessary follow-up from nonspecific findings. That position does not make whole-body MRI an unreasonable personal choice. It supports selecting patients thoughtfully and discussing the possible consequences of incidental findings before testing.

Similar caution applies to multi-cancer detection blood tests. The National Cancer Institute continues to note that these tests have not yet been shown in randomized trials to reduce cancer mortality and should not replace established cancer screening.

The HormoneSynergy® Approach

HormoneSynergy® is structured around the clinical question rather than a fixed requirement that every person complete the largest available testing package. Some people want a DEXA scan to understand bone density and visceral fat, a SECA body composition assessment, carotid ultrasound or a focused evaluation of coronary plaque. Others want a physician to bring cardiovascular risk, metabolism, hormones, bone health, body composition and cognition together in one medical assessment.

For patients seeking comprehensive physician-guided care, the starting point is our Optimal Aging Assessment. The assessment is currently $2,995 and combines advanced laboratory testing with carotid ultrasound, Hologic® DEXA for bone density and visceral fat, SECA body composition analysis, objective neurocognitive testing and a 2–3 hour consultation with Dr. Kathryn Retzler.

Patients leave that consultation with an individualized medical assessment and treatment plan. Hormone therapy, cardiovascular risk management, metabolic treatment and other options may be discussed according to the history and findings.

After the assessment, a patient may take the plan to another physician or continue through our concierge practice. Patients who want Dr. Retzler to prescribe, monitor treatment and provide ongoing medical management must be enrolled in concierge care. There is no contract. Membership is $150 per month and is paid three months at a time.

Concierge membership includes direct EMR communication, annual neurocognitive testing, annual SECA body composition testing and $100 in supplement credit for each active month. That equals $300 during each three-month membership period and $1,200 over 12 months. Physician visits and laboratory work are billed separately according to time, complexity and clinical need.

Focused Testing Without Comprehensive Care

A comprehensive assessment is useful when the clinical questions cross several areas of health, but it is not necessary for every person who wants a particular test. Selected services such as DEXA, SECA, carotid ultrasound and stand-alone Cleerly® coronary plaque evaluation remain available separately when appropriate.

For someone already completing the Optimal Aging Assessment, Cleerly® CCTA plaque analysis may be added when appropriate for $995, with the imaging center typically charging another $1,300–$1,400 directly for the coronary CT angiogram. The stand-alone pathway is $1,995 through HormoneSynergy®, including the Cleerly analysis and physician review, plus the imaging center’s charge.

This structure allows the evaluation to remain proportional to the question. A focused test may be sufficient, while findings that reveal a broader problem may justify expanding the evaluation.

What Happens After Testing?

Before ordering another biomarker, scan or genetic panel, it is worth asking what the result could reasonably change. ApoB, blood pressure, bone loss, visceral fat, insulin resistance and coronary plaque all have established clinical implications. Hormone testing can also be actionable when interpreted in the context of symptoms, life stage, medications and medical history.

Other findings may contribute to the overall picture without carrying the same clinical weight. A biological-age estimate, an isolated microbiome pattern or a small deviation in an uncommon biomarker does not automatically require treatment. Some findings deserve investigation, some deserve observation and others have little practical importance for that particular patient.

A large report may identify elevated ApoB, high lipoprotein(a), borderline blood pressure, low ferritin, several hormone changes and a mildly abnormal autoimmune marker. Those findings do not carry equal risk or deserve equal attention. Family history, symptoms, medications, age, previous testing and personal goals determine how they should be prioritized.

The physician must then decide whether cardiovascular risk justifies medication, whether coronary imaging would add useful information, whether hormone therapy is reasonable, whether low ferritin warrants further evaluation and which tests need to be repeated. Generating health data has become easier; deciding what deserves medical attention remains the more demanding part of the work.

Cost and Ongoing Care

Price alone cannot identify the best value because these programs provide different services. The following figures make the underlying assumptions more visible.

On a smaller screen, swipe horizontally to compare.

Program Published or typical cost What the figure represents
Function Health® $365/year Annual laboratory membership. Imaging, prescriptions and medical management by the patient’s physician are separate
HormoneSynergy® Typical first year: $5,600–$6,000

Typical later year: approximately $3,000
The first-year estimate includes the $2,995 assessment, 12 months of concierge membership totaling $1,800 and two to three typical follow-up visits totaling approximately $800–$1,200. A typical later year includes 12 months of membership and three visits totaling approximately $1,200. Active membership provides $100 per month in supplement credit, or $1,200 over 12 months
Fountain Life™ CORE: $10,500/year

APEX: $21,500/year
Published membership prices. À-la-carte diagnostics, restorative therapies, prescriptions, treatment prompted by findings, specialist care and travel may add to the total

Among physician-led longevity programs with published pricing, HormoneSynergy® sits toward the more affordable end while providing a comprehensive evaluation of cardiovascular risk, metabolism, hormones, bone health, body composition and cognition. The lower price does not reflect a limited assessment. It reflects a more selective approach to advanced diagnostics. Whole-body MRI, genomic testing, VO₂ max and other specialized studies can be added when they are likely to provide useful information rather than being bundled into every patient’s initial cost.

HormoneSynergy® has no contract for concierge membership. Payments cover three months at a time, currently $450. The annual estimates above are intended to show what a patient who remains enrolled for 12 months typically spends, not to create an annual commitment.

Fountain Life™ also lists individual tests and services separately. Current examples include a multi-cancer early-detection blood test at $949, microbiome testing at $485, targeted MRI at $500 and extended vitamin and nutrient testing at $175. Additional imaging, prescriptions, specialist evaluation and treatment of abnormalities may increase the total further, as may travel and lodging for patients who do not live near a Fountain Life™ center.

HormoneSynergy® does not bill insurance for preventive longevity medicine. We also do not mark up or receive profit from recommended outside laboratory testing, imaging studies or pharmaceutical prescriptions. Those fees are paid directly to the laboratory, imaging center or pharmacy. Our broader approach is explained in Insurance Is Not Healthcare: Our Approach to Preventive Longevity Medicine.

Choosing the Right Approach

Strong family histories of premature cardiovascular disease or cancer may justify a broader evaluation. So may unexplained symptoms, significant cognitive changes, rapidly worsening metabolic health, known coronary calcium, osteoporosis or previous findings that require surveillance. Healthy people may also choose extensive screening after considering both the potential benefit and the possibility of incidental findings.

Function Health® may be an excellent fit for someone who wants broad laboratory surveillance and already has a physician prepared to manage the results. Fountain Life™ may appeal to someone seeking a technologically extensive early-detection program and willing to accept the cost and possible downstream evaluation. HormoneSynergy® may fit someone who wants testing integrated into a physician-led assessment, with the option to continue into ongoing care or use focused diagnostic services separately.

Already Have Function Health® or Fountain Life™ Results?

Useful outside testing does not need to be repeated merely because it originated elsewhere. Patients may bring recent Function Health® laboratories, Fountain Life™ imaging or testing from another qualified laboratory or medical center. We review what has already been completed, identify what remains useful to evaluate and incorporate appropriate outside information into the medical assessment.

Longevity medicine should remain open to useful technology without assuming that every available measurement belongs in every person’s workup. The appropriate amount of testing is the amount that improves medical decisions and provides a responsible plan for what happens next.
