Supplements for Menopause — What May Help, What’s Overhyped and What to Measure
Menopause creates an unusually fertile market for supplements. Sleep changes. Body composition can shift. Bone loss accelerates. Maintaining muscle becomes more difficult. Hot flashes, mood changes, fatigue, joint symptoms and cognitive complaints may emerge at roughly the same time.
It is hardly surprising that many women begin looking for something they can take.
Some supplements do have a useful place during this transition, although their most defensible roles are usually less dramatic than the advertising suggests. Calcium can help close a dietary gap. Vitamin D remains important when intake or clinical circumstances justify supplementation. Magnesium can be useful when dietary intake is inadequate. Creatine has increasingly good evidence for supporting muscle and strength, particularly when it accompanies resistance training. Omega-3 fatty acids can improve EPA and DHA intake and have an established place in cardiometabolic nutrition.
That is different from claiming that a supplement “balances hormones,” restores estrogen, treats every menopause symptom or replaces effective menopause therapy.
A more useful approach begins by identifying the problem we are actually trying to solve, selecting an intervention that fits that problem, and measuring the outcome when measurement can influence the next decision.

There is no universal menopause supplement stack. The supplements most likely to earn a place are the ones tied to a real nutritional or physiologic need.
Calcium and vitamin D remain important for skeletal health, but the objective is adequate intake rather than automatically taking large doses. Magnesium may be useful when dietary intake is low or when a well-tolerated supplemental source is needed. Creatine has increasingly useful evidence in postmenopausal women, especially when combined with resistance training, with studies suggesting modest improvements in lean mass and strength. Omega-3 supplements can help women who eat little fatty fish and may have additional value when triglycerides are elevated.
Soy foods are nutritious and may modestly help some women. Evidence for concentrated soy isoflavones and black cohosh remains mixed. Products marketed as hormone balancers, adrenal support, wild-yam progesterone or natural estrogen replacement usually promise more than the clinical evidence supports.
Measurement is often more useful than another bottle. Bone density, lean mass, blood pressure, lipids, glucose regulation and selected nutrient testing can reveal far more about healthy aging than a large hormone panel. In otherwise healthy women over 45 with typical menopause symptoms, routine FSH and estradiol testing is generally unnecessary for diagnosing menopause.
Menopause Changes More Than Menstrual Cycles
Nutrition deserves more attention during menopause because the physiology surrounding it is changing as well.
Estrogen participates in the regulation of bone remodeling. As estrogen declines, bone resorption increases, and bone density can fall more rapidly during the menopausal transition and the years immediately afterward.
Muscle deserves similar attention. Aging itself makes skeletal muscle more difficult to preserve, while the menopause years often coincide with changes in sleep, physical activity, protein intake and body composition that may further accelerate the loss of lean mass.
Visceral fat may increase even when body weight changes very little. Lipids, insulin sensitivity and blood pressure can shift through midlife as well.
For that reason, a thoughtful menopause supplement strategy should extend well beyond hot flashes. Preserving bone, muscle and metabolic health over the next twenty years may be considerably more important than finding a capsule that produces a small short-term reduction in one symptom.

Supplements and Hormone Therapy Do Different Jobs
Supplements can support nutrition. They are not interchangeable with menopausal hormone therapy.
The Menopause Society continues to identify hormone therapy as the most effective treatment for bothersome vasomotor symptoms, including hot flashes and night sweats. Hormone therapy can also prevent menopause-related bone loss and reduce fracture risk in appropriately selected women.
That does not mean hormone therapy is appropriate or desirable for every woman. Medical history, symptoms, age, time since menopause, individual risk and personal preference all belong in the decision.
It does mean that a botanical supplement should not be presented as though it offers the same established effects as appropriately prescribed estrogen or progesterone.
Women who cannot use hormone therapy, or who prefer not to use it, also have evidence-based nonhormonal treatment options. Supplements may be part of that discussion, but they are not the only alternative.
Creatine Has Become One of the More Interesting Menopause Supplements
Creatine was once discussed almost entirely in the context of sports performance. That view has broadened considerably.
Creatine supports rapid ATP regeneration in skeletal muscle, improving the muscle's ability to generate force and support training. That becomes increasingly relevant as preserving lean mass, strength and physical function takes on greater importance with age.
A 2026 systematic review and meta-analysis examined seven randomized controlled trials involving more than 600 postmenopausal women. Creatine supplementation produced a small but statistically significant increase in lean mass and improved leg-press strength. The clearest benefits appeared when at least 5 grams per day was combined with resistance training.
The findings for bone were less compelling. Overall bone mineral density did not improve significantly, although individual studies have reported potentially favorable changes in bone geometry or preservation of selected skeletal measures.
Creatine should not be sold as an osteoporosis treatment. Its stronger role in menopause is on the muscle side of healthy aging, particularly when resistance training is already part of the plan.
That has consequences well beyond appearance. Stronger muscle contributes to glucose disposal, balance, physical function, skeletal loading and the ability to remain independent later in life.

Protein May Matter More Than Most Menopause Supplements
Protein powder is technically a supplement, although its role bears little resemblance to that of a botanical menopause formula.
A woman who consistently meets her protein needs through food has little reason to begin using protein powder simply because menopause has started. For a woman who routinely falls short, however, protein powder may be one of the most practical additions available.
The goal is straightforward: support muscle protein synthesis, recovery from resistance training and preservation of lean mass.

Protein supplementation is most useful when it closes a genuine dietary gap rather than simply adding more protein to an already adequate diet.
Muscle preservation becomes especially important during intentional weight loss, including GLP-1-assisted weight loss, because the weight being lost can include lean tissue when protein intake and resistance training are neglected.
Calcium: Meet the Requirement, Then Stop Adding
Calcium is essential for bone mineralization, and recommended calcium intake rises for women beginning at age 51.
The general target is approximately 1,200 mg per day from food and supplements combined.
The combined total is important. A woman who regularly consumes yogurt, cheese, fortified plant milk and other calcium-rich foods may already be close to the recommended intake before opening a supplement bottle.
The goal is not to take 1,200 mg of supplemental calcium. The goal is to reach approximately 1,200 mg in total.
If food provides 800 or 900 mg, a modest supplement may be enough to close the gap. If the diet already supplies the full amount, there is no clear advantage to adding another 1,200 mg on top of it.
Calcium citrate can be useful when stomach acid is reduced or acid-suppressing medications are being used. Calcium carbonate can also be effective, although it is generally absorbed best when taken with food.

Vitamin D Still Matters, but the Goal Has Become More Nuanced
Vitamin D supports calcium absorption, bone mineralization and normal muscle function.
What has changed is the assumption that every healthy woman should automatically take a large dose or repeatedly measure vitamin D simply in the name of prevention.
The Endocrine Society's 2024 guideline recommends that generally healthy adults ages 50 through 74 follow established dietary requirements rather than routinely supplementing above them. The guideline also recommends against routine vitamin D testing in otherwise healthy adults without another clinical reason for testing.
None of this makes vitamin D deficiency unimportant. Osteoporosis, malabsorption, hypocalcemia, kidney disease, certain medications and other clinical circumstances can substantially change the decision.
The dose should have a reason behind it. A woman being treated for osteoporosis or documented vitamin D deficiency is in a very different situation from a healthy 52-year-old taking 5,000 IU every day solely because menopause has begun.

Magnesium Can Be Useful Without Calling It a Menopause Treatment
Magnesium participates in muscle and nerve function, glucose regulation, blood pressure control, energy metabolism and bone physiology.
Many women do not consistently consume magnesium-rich foods such as beans, nuts, seeds, whole grains and leafy vegetables. Supplementation can make sense when dietary intake is inadequate or when a particular formulation offers a practical way to meet that need.
Magnesium is also commonly used for sleep and relaxation. Clinical research suggests some benefit in selected people with poor sleep, but the effects are generally modest. Magnesium should not be presented as a treatment for every form of menopausal insomnia.
Hot flashes, night sweats, obstructive sleep apnea, alcohol, restless legs, medications and circadian disruption can all interfere with sleep during menopause. No magnesium capsule can correct all of those problems.
Formulation affects both absorption and gastrointestinal tolerance. Glycinate-based forms can be useful when a gentle general supplement is desired, while citrate may be reasonable when bowel regularity is also a goal.

Omega-3s Fit the Cardiometabolic Side of Menopause
Omega-3 fish oil sometimes appears in menopause formulas as though it were primarily a treatment for hot flashes. That is not where the evidence is strongest.
EPA and DHA have established roles in triglyceride metabolism and cardiovascular nutrition. A woman who rarely eats fatty fish may reasonably use a high-quality omega-3 supplement to increase her intake of EPA and DHA.
Higher-dose prescription omega-3 therapy can substantially reduce triglycerides in appropriately selected patients. Ordinary nutritional fish-oil supplements should not be confused with prescription treatment for elevated triglycerides.
The Menopause Society's 2023 review did not find omega-3 supplements sufficiently effective for vasomotor symptoms to recommend them as a hot-flash treatment.
That finding does not make omega-3 supplementation irrelevant during menopause. It simply places it in the appropriate clinical context. Its strongest potential value is nutritional and cardiometabolic rather than hormonal.

Soy Foods Remain a Reasonable Part of the Conversation
Soy has spent decades caught in a polarized discussion about menopause that rarely reflects the nuance of the evidence.
Soy foods such as tofu, tempeh, edamame and unsweetened soy milk can provide protein, minerals and isoflavones as part of an otherwise healthy diet.
Concentrated isoflavone supplements are a more complicated question. Earlier meta-analyses suggested modest reductions in the frequency and severity of hot flashes. More recent reviews have been mixed, including analyses that did not find a significant improvement in vasomotor symptoms.
NICE menopause guidance takes a practical middle position. There is some evidence that isoflavones may improve vasomotor symptoms, but formulations vary, and safety and medication interactions still need to be considered.
The Menopause Society's 2023 nonhormone therapy statement was less enthusiastic, declining to recommend supplements or herbal remedies as established treatments for vasomotor symptoms because the overall evidence remains inconsistent.
For women who enjoy soy foods, none of this requires an all-or-nothing decision. Soy can remain part of a plant-forward, protein-forward diet even when a concentrated soy supplement is not being used as a treatment for hot flashes.
Black Cohosh Has Some Signal, but Product Quality Matters
Black cohosh is probably the botanical most closely associated with menopause.
The evidence is mixed rather than uniformly negative. NCCIH notes that a 2023 review of 22 studies found potentially favorable effects on overall menopause symptoms and some improvement in hot flashes. Other reviews and clinical guidelines have judged the evidence too inconsistent to support routine use.
Part of the uncertainty comes from the products themselves. “Black cohosh” does not always describe the same intervention. Studies have used different extracts, doses and combination products. Commercial testing has also identified products containing the wrong plant or ingredients that were not disclosed on the label.
Black cohosh has generally been well tolerated in shorter clinical studies, although rare reports of serious liver injury have been associated with products labeled as black cohosh. In many of those cases, it has remained uncertain whether black cohosh itself was responsible.
We therefore view black cohosh as a possible option for selected women rather than a foundation of menopause care. A standardized, reputable product and a thoughtful medication review are more important than the words “natural menopause support” on the front of a bottle.
Red Clover and Other Phytoestrogen Products Have Less Convincing Evidence
Red clover contains isoflavones and has been marketed for hot flashes, cardiovascular health and bone support.
Clinical studies have produced inconsistent findings for vasomotor symptoms, cholesterol and bone density. NCCIH concludes that the evidence remains insufficient to support firm recommendations.
That does not mean red clover can never be considered. It means expectations should remain modest, and the product should not be presented as a proven substitute for hormone therapy.
Wild Yam Does Not Become Progesterone in the Human Body
Wild-yam creams illustrate how a legitimate biochemical fact can be transformed into a much broader marketing claim.
Wild yam contains diosgenin, a compound that can be used as a starting material in a laboratory to manufacture steroid hormones. Human cells, however, do not possess the biochemical pathway needed to convert wild-yam diosgenin into progesterone.
The Menopause Society's review found no convincing evidence that wild-yam creams improve vasomotor symptoms. Some products tested as wild-yam preparations have also been found to contain undisclosed hormones.
A commercial wild-yam cream should therefore not be regarded as “natural progesterone.” Prescription micronized progesterone is an actual hormone with a defined dose and an established clinical role. They are not interchangeable.
DHEA Requires the Same Distinction
DHEA is a hormone precursor produced naturally by the adrenal glands and is available over the counter in the United States.
Systemic DHEA supplements have been marketed for energy, libido, body composition, mood and menopause symptoms. Available evidence has not established routine oral DHEA as an effective treatment for menopause.
DHEA can be converted into both androgens and estrogens in the body, which is one reason indiscriminate dosing is difficult to justify.
Prescription vaginal prasterone is a different therapy. It is FDA-approved for dyspareunia associated with genitourinary syndrome of menopause. An over-the-counter oral DHEA supplement should not be conflated with that prescription treatment.
“Adrenal Support” and “Hormone Balance” Need a Defined Target
Menopause products increasingly combine adaptogens, glandular extracts, herbal blends and nutrients under broad labels such as adrenal support, cortisol balance or female hormone balance.
Some of the individual ingredients may have legitimate physiologic effects. The difficulty lies in the breadth of what the finished product is often claimed to accomplish.
Menopause is not an undefined failure of “hormone balance.” It is a recognizable hormonal transition involving changing ovarian estrogen and progesterone production, with effects that extend across multiple organ systems.
If the problem is hot flashes, the clinical question is how to address vasomotor symptoms. If the problem is sleep, we need to understand what is disturbing sleep. If bone density is falling, it should be measured. If muscle is being lost, protein intake, resistance training and body composition deserve attention.
A supplement is much easier to judge when it has a specific job.
Estrogen-Metabolism Supplements Are a Separate Question
DIM, indole-3-carbinol, sulforaphane, glutathione-support nutrients and related products are sometimes discussed during menopause because of their relationship to estrogen metabolism.
These compounds should not be marketed as treatments for hot flashes or as substitutes for estrogen.
They may be considered in selected nutritional or estrogen-metabolism contexts, but that is a different clinical question from treating menopausal symptoms.
Supporting a biochemical pathway is not the same as correcting or replacing the hormonal physiology of menopause.
What About Collagen?
Collagen peptides have legitimate research interest in connective tissue, skin and bone, and certain patented collagen peptides have been studied specifically in postmenopausal women.
When the primary goal is preserving muscle, however, we still place adequate total protein and resistance training ahead of collagen.
Collagen can serve as an adjunct when skin, joint or skeletal support is part of the goal. It is not a complete protein replacement and should not displace the dietary protein needed to preserve lean mass.
What Is Actually Worth Measuring During Menopause?
The measurements worth ordering are the ones capable of changing care.
For most otherwise healthy women over 45 with typical symptoms and menstrual changes, menopause itself is a clinical diagnosis. NICE guidance specifically advises against routinely using FSH, estradiol, anti-Müllerian hormone and several other ovarian tests to diagnose perimenopause or menopause in this age group.
Hormone concentrations can fluctuate substantially during perimenopause. A single estradiol or FSH result may therefore create an appearance of precision without providing information that meaningfully changes management.
Testing still has an important role when the clinical question is different. Women under 45 with possible early menopause, women under 40 with suspected premature ovarian insufficiency, women with unusual bleeding patterns and patients with complex endocrine presentations may require a different evaluation.
Bone Density Is One of the Most Valuable Measurements
Bone loss accelerates as estrogen declines, and a calcium supplement cannot tell us whether that loss is actually taking place.
DEXA can.
The 2025 U.S. Preventive Services Task Force recommendation advises osteoporosis screening for all women age 65 and older. It also recommends screening postmenopausal women younger than 65 when clinical risk assessment indicates increased fracture risk.
DEXA may also be clinically useful earlier in selected women, particularly when low body weight, family history, previous fracture, glucocorticoid exposure, early menopause, significant dietary restriction or another risk factor raises concern about lower bone mass.

Lean Mass Deserves to Be Followed Too
Bone density appropriately receives considerable attention during menopause, but lean mass is also central to healthy aging.
A woman can remain at roughly the same body weight while gradually losing muscle and accumulating visceral fat. A bathroom scale will not reveal that change.
DEXA body composition and other validated body-composition measurements can help determine whether resistance training, protein intake and weight-loss strategies are preserving lean tissue.
This is where creatine and protein become more clinically interesting. When body-composition testing identifies an actual muscle problem, the supplement decision is tied to something that can be followed rather than simply added by habit.
Cardiometabolic Markers Belong in the Menopause Conversation
Cardiovascular prevention in midlife should not be reduced to products in a menopause supplement aisle.
Blood pressure, fasting glucose or hemoglobin A1c, triglycerides, LDL-related risk and body composition deserve attention. At HormoneSynergy®, we also place substantial emphasis on apoB because it more directly reflects the number of circulating atherogenic lipoprotein particles.
Lipoprotein(a) is generally worth measuring at least once because it is largely genetically determined and can meaningfully alter lifetime cardiovascular risk assessment.
If triglycerides are elevated, a discussion about omega-3s becomes more relevant. If visceral fat and insulin resistance are increasing, the larger plan should address those findings directly rather than relying on a supplement marketed for “menopause metabolism.”
Iron Testing Should Follow the Menstrual History
Perimenopause can bring heavier or less predictable bleeding, creating iron deficiency in women who previously maintained normal iron stores.
Fatigue, hair shedding and reduced exercise tolerance are often attributed to menopause, yet ferritin and a complete blood count may reveal iron depletion when menstrual blood loss has increased.
Once menstruation ends, routine iron requirements decline substantially. New iron deficiency in a postmenopausal woman deserves an explanation rather than automatic long-term iron supplementation.

Vitamin B12 Testing Is Useful When Risk Factors Are Present
Vitamin B12 deficiency can produce fatigue, neuropathy, balance problems and cognitive symptoms, all of which can overlap with complaints commonly attributed to menopause.
Testing becomes particularly useful in women taking metformin or long-term acid-suppressing medications, those following vegan diets, and women with gastrointestinal disease or previous bariatric surgery.
A borderline serum B12 result may sometimes require methylmalonic acid testing to clarify whether a functional deficiency is present.
Menopause does not make every symptom hormonal, which is precisely why overlapping causes deserve consideration.

Thyroid Testing Has a Role When the Symptoms Fit
Fatigue, sleep disruption, weight change, hair changes, mood symptoms, palpitations and temperature sensitivity can all occur during menopause. Several of the same complaints can also occur with thyroid disease.
Routine thyroid testing is not required simply because a woman is menopausal, but TSH and related testing can be appropriate when the history or symptom pattern makes thyroid dysfunction a reasonable alternative explanation.
Good menopause medicine does not require assigning every midlife symptom to estrogen.
Vitamin D Testing Should Be Selective Rather Than Automatic
Vitamin D became one of the most routinely ordered nutritional tests in medicine.
The Endocrine Society's updated guidance now recommends against routine vitamin D screening in generally healthy adults when there is no clinical indication for testing.
There are still many women for whom vitamin D testing is reasonable, including selected patients with osteoporosis, malabsorption, hypocalcemia and other medical circumstances in which the result can influence treatment.
The deciding question is whether the result is likely to change management. Vitamin D remains biologically important regardless of whether a blood test is needed.

A menopause plan should ultimately leave a woman healthier, stronger and better informed about her future risk. Supplements can support that plan when there is a clear reason for each one to be there.

What are the best supplements for menopause?
There is no universal menopause supplement list. Calcium, vitamin D, magnesium, creatine, protein and omega-3s may be useful depending on diet, bone health, body composition and clinical needs. Botanical products such as soy isoflavones or black cohosh have less consistent evidence for menopause symptoms.
Does creatine help women after menopause?
Evidence is increasingly supportive for muscle and strength. A 2026 meta-analysis of randomized trials found small improvements in lean mass and strength in postmenopausal women, with the clearest benefits when creatine at approximately 5 grams per day or more was combined with resistance training. Effects on bone mineral density remain uncertain.
Should women take calcium during menopause?
Women age 51 and older generally need about 1,200 mg of calcium per day from food and supplements combined. A supplement is most useful when food intake does not provide enough calcium. The full 1,200 mg does not need to come from a supplement.
Should every menopausal woman take vitamin D?
Women should obtain adequate vitamin D, but healthy adults under age 75 are not routinely advised to take doses above established dietary requirements simply for disease prevention. Higher doses and testing may be appropriate when osteoporosis, deficiency, malabsorption or another clinical indication is present.
Does magnesium help menopause symptoms?
Magnesium supports normal muscle, nerve, bone and metabolic function and may be useful when dietary intake is inadequate. It may modestly support sleep in some people, but it has not been established as a reliable treatment for hot flashes or the full range of menopause symptoms.
Do soy isoflavones help hot flashes?
Research is mixed. Some studies and older meta-analyses have reported modest reductions in hot flashes, while more recent analyses have not consistently confirmed a vasomotor benefit. Soy foods remain nutritious regardless of whether concentrated isoflavone supplements are used.
Does black cohosh work for hot flashes?
Some studies and reviews suggest that selected black cohosh extracts may reduce menopause symptoms, while other reviews and clinical guidelines find the evidence inconsistent. Product quality varies, and rare reports of liver injury have been associated with products labeled as black cohosh.
Does wild yam increase progesterone?
No. Wild yam contains diosgenin, which can be converted into steroid hormones in a laboratory, but the human body cannot convert wild-yam diosgenin into progesterone. Wild-yam cream should not be treated as equivalent to prescription micronized progesterone.
Do I need hormone testing to know whether I am in menopause?
Usually not if you are over 45 and otherwise healthy with typical menopause symptoms and menstrual changes. Hormone levels fluctuate substantially during perimenopause, and guidelines generally recommend clinical diagnosis rather than routine FSH or estradiol testing in this age group.
What should women measure during menopause?
Useful measurements depend on individual risk but may include blood pressure, lipids and apoB, glucose or A1c, body composition and bone density. DEXA screening is recommended for women age 65 and older and for younger postmenopausal women at increased fracture risk. Iron, B12, thyroid and vitamin D testing may be appropriate when symptoms, medications or medical history create a specific reason to test.
Can supplements replace menopause hormone therapy?
No supplement has been shown to reproduce the full effects of appropriately prescribed menopausal hormone therapy. Hormone therapy remains the most effective treatment for vasomotor symptoms and can prevent bone loss in appropriately selected women. Supplements can still have useful nutritional and supportive roles within a broader menopause plan.
