Supplements and Prescription Medications — Common Interactions People Miss
Supplements tend to disappear from the medication list.
A patient may carefully report levothyroxine, losartan, metformin and an antibiotic while forgetting to mention the calcium tablet, magnesium powder, multivitamin, potassium product, B-complex and herbal sleep formula used every day. The supplements feel like nutrition rather than medication.
Pharmacologically, that distinction is not always useful.
Minerals can bind certain medications in the intestine and reduce absorption. Vitamin K can alter the effect of warfarin. Potassium supplements can become problematic when combined with drugs that already raise potassium. St. John’s wort can accelerate the metabolism of numerous prescription medications. High-dose biotin can distort laboratory results without changing the medication itself.
The interaction can also run in the other direction. Metformin can lower vitamin B12 status. Long-term proton-pump inhibitor use can affect B12 and magnesium. Diuretics can raise or lower mineral losses depending on the drug.
A supplement review therefore belongs inside a medication review, not beside it.

Some of the most important supplement-medication interactions involve ordinary minerals. Calcium and iron can reduce levothyroxine absorption. Calcium, magnesium, iron and zinc can bind certain antibiotics and substantially reduce drug absorption. Ciprofloxacin, for example, is generally taken at least two hours before or six hours after products containing these minerals.
Vitamin K requires consistent intake in people taking warfarin. Potassium deserves caution with ACE inhibitors, ARBs and potassium-sparing diuretics such as spironolactone because the combination can increase the risk of hyperkalemia. High-dose vitamin E may increase bleeding risk when combined with anticoagulant or antiplatelet medications.
St. John’s wort has one of the most extensive interaction profiles among herbal supplements. It can reduce the effectiveness of oral contraceptives, warfarin, transplant medications, some statins, HIV medications and certain cancer drugs. Combining it with antidepressants can also increase serious serotonin-related adverse effects.
Prescription medications can affect nutrients as well. Metformin and long-term acid suppression can lower B12 status. Proton-pump inhibitors can occasionally cause significant magnesium depletion. Diuretics can increase or decrease potassium and magnesium losses depending on the medication.
The safest approach is simple: every supplement, powder, gummy and herbal product belongs on the medication list.
Thyroid Medication and Minerals
Levothyroxine is one of the clearest examples of why supplement timing matters.
Calcium can bind levothyroxine in the gastrointestinal tract and reduce the amount absorbed. Iron can do the same. The result may be a higher TSH or the appearance that the thyroid dose is no longer adequate even though the problem is administration rather than the medication itself.
FDA labeling and NIH guidance generally advise separating levothyroxine from calcium and iron by at least four hours. The NIH Calcium Fact Sheet and NIH Iron Fact Sheet both describe these interactions.
This becomes easy to miss when calcium or iron is hidden inside a multivitamin, prenatal formula, bone product or meal replacement rather than taken as a standalone tablet.
Consistency also matters. Someone who has taken levothyroxine and a multivitamin together for years may have had the thyroid dose adjusted around that routine. Changing the timing can change absorption and should be communicated to the prescribing clinician.
Antibiotics and Mineral Supplements
Several antibiotics interact directly with mineral supplements.
Tetracyclines and fluoroquinolones can bind calcium, magnesium, iron and zinc inside the gastrointestinal tract. The resulting complex is poorly absorbed, which means less antibiotic reaches the circulation.
Magnesium-containing supplements should generally be separated from tetracycline and quinolone antibiotics by several hours. NIH guidance advises taking many of these antibiotics at least two hours before or four to six hours after magnesium.
Ciprofloxacin labeling is particularly specific. Products containing calcium, magnesium, iron or zinc are generally taken at least two hours before or six hours after ciprofloxacin. The current DailyMed ciprofloxacin prescribing information includes this administration requirement.
This applies to multivitamins and mineral combination products as well as standalone supplements.
Someone being treated for an infection should follow the timing instructions for the specific antibiotic rather than relying on a general supplement schedule.
Warfarin and Vitamin K
Vitamin K and warfarin are frequently described as substances that should never be used together. The more accurate advice is that vitamin K intake should remain reasonably consistent.
Warfarin works by interfering with vitamin K-dependent clotting factors. A sudden increase in vitamin K intake can reduce anticoagulation. A sudden decrease can increase it.
This includes vitamin K coming from supplements. Starting a multivitamin, vitamin K2 supplement or bone-health formula can change the amount of vitamin K consumed each day.
The NIH Vitamin K Fact Sheet advises people taking warfarin to maintain a consistent intake from both food and supplements rather than avoiding vitamin K completely.
Any meaningful change in supplemental vitamin K should be discussed with the clinician managing warfarin and INR monitoring.
Potassium Supplements With ACE Inhibitors, ARBs and Spironolactone
Potassium is another nutrient that can behave very differently depending on the medication list.
ACE inhibitors such as lisinopril and benazepril, and angiotensin receptor blockers such as losartan and valsartan, can reduce urinary potassium loss. Potassium-sparing diuretics such as spironolactone and amiloride have a similar effect.
Adding potassium supplements or potassium-containing salt substitutes can raise potassium further, particularly in people with impaired kidney function.
Hyperkalemia can become clinically serious because potassium is central to cardiac electrical activity. The NIH Potassium Fact Sheet recommends monitoring potassium status in people taking ACE inhibitors, ARBs and potassium-sparing diuretics when other risk factors are present.
The opposite problem occurs with loop and thiazide diuretics. Drugs such as furosemide and hydrochlorothiazide can increase urinary potassium losses and contribute to low potassium in some patients.
The medication determines whether potassium supplementation is potentially useful or potentially hazardous.
St. John’s Wort Is Not a Mild Interaction
St. John’s wort deserves special treatment because its interaction list is unusually broad.
The herb activates drug-metabolizing enzymes and intestinal transport proteins, allowing some medications to be cleared from the body more rapidly. Drug concentrations can fall enough to reduce effectiveness.
The National Center for Complementary and Integrative Health lists documented interactions with oral contraceptives, warfarin, cyclosporine, digoxin, certain statins, some HIV medications, seizure medications and several cancer therapies. The NCCIH St. John’s Wort review describes these interactions in detail.
This is especially important with transplant medications and cancer therapies, where reduced drug exposure can have serious consequences.
St. John’s wort can also interact with antidepressants through serotonin pathways. Combining it with medications that already increase serotonin can produce serious serotonin-related adverse effects.
“Natural antidepressant” is therefore not an adequate safety description for St. John’s wort.
Vitamin E and Blood-Thinning Medications
Vitamin E is essential at normal nutritional intake, but large supplemental doses can inhibit platelet aggregation and affect vitamin K-dependent clotting.
When high-dose vitamin E is combined with anticoagulant or antiplatelet medications, bleeding risk may increase. The amount required to create a clinically important interaction is not precisely defined, although NIH notes that the doses involved are probably greater than 400 IU per day.
The concern is high-dose supplementation, not vitamin E naturally present in nuts, seeds and other foods.
The NIH Vitamin E Fact Sheet specifically advises people taking anticoagulant or antiplatelet medications to review supplemental vitamin E with their healthcare professional.
Iron Can Reduce Levodopa Absorption
Iron interacts with more than thyroid medication.
Iron supplements can reduce absorption of levodopa, a medication used in Parkinson’s disease and in some cases of restless legs syndrome. Less absorbed levodopa can mean less medication effect.
This is easy to overlook when a patient with restless legs is also found to have low ferritin. Iron may be clinically appropriate, but the iron and levodopa schedule still needs to be coordinated.

Metformin Can Lower Vitamin B12
Not every interaction means that a supplement changes a drug. Sometimes the medication changes nutrient status.
Metformin can reduce vitamin B12 absorption and lower serum B12 concentrations. The risk becomes more relevant with long-term use and in people who already have other reasons for poor B12 absorption.
This matters because B12 deficiency can cause peripheral neuropathy. In someone with diabetes, numbness or tingling can easily be attributed to diabetic neuropathy even when B12 deficiency is contributing.

Proton-Pump Inhibitors Can Affect B12 and Magnesium
Proton-pump inhibitors are effective medications for several gastrointestinal conditions and are widely used. Long-term therapy can also influence nutrient physiology.
Reducing stomach acid can make it harder to release vitamin B12 from food protein. NIH identifies long-term gastric acid suppression as a risk factor for reduced B12 absorption.
PPIs can also cause hypomagnesemia in some long-term users. The effect is uncommon but can become clinically significant, particularly when other factors also lower magnesium.
The NIH Magnesium Fact Sheet notes that prolonged prescription PPI use has produced cases of low magnesium severe enough that supplementation alone did not always correct the problem.
This does not mean everyone using omeprazole or another PPI needs routine B12 and magnesium supplementation. It means duration of therapy and symptoms belong in the nutritional history.
Diuretics Can Move Magnesium and Potassium in Different Directions
The word diuretic covers several drug classes, and they do not all affect minerals the same way.
Loop diuretics such as furosemide and thiazide-type diuretics can increase urinary magnesium losses. They can also increase potassium losses and contribute to hypokalemia.
Potassium-sparing diuretics such as spironolactone and amiloride do the opposite with potassium and also reduce magnesium excretion.
This becomes important when someone begins supplementing magnesium or potassium based on the assumption that all diuretics “deplete electrolytes.” The specific medication matters.
Biotin Can Change the Laboratory Result Without Changing the Patient
Biotin creates one of the strangest supplement-medication problems because the drug itself may be working normally while the laboratory result appears abnormal.
High-dose biotin is common in hair, skin and nail supplements and in some B-complex products. Certain laboratory tests use biotin-based technology, and elevated biotin concentrations can generate falsely high or falsely low results.
Thyroid testing is a well-known example. Someone taking large amounts of biotin can produce a laboratory pattern that looks like thyroid disease even when thyroid physiology is normal.
The FDA has also continued to warn about falsely low troponin results in certain assays used to evaluate suspected heart attacks. Its biotin laboratory-interference guidance remains an important safety reference.
A patient may never think to mention a hair supplement before thyroid or cardiac testing unless someone asks.
A Multivitamin Can Be Part of the Interaction
People often report standalone supplements while forgetting that their multivitamin contains many of the same ingredients.
Calcium, iron, magnesium, zinc, vitamin K and biotin can all appear in combination products. A single daily multivitamin can therefore affect medication timing even when no standalone mineral supplement is being used.
This does not make multivitamins inherently problematic. It means their ingredient list belongs in the same medication reconciliation as every other supplement.

Combination Products Make Interactions Easier to Miss
The modern supplement aisle is organized around outcomes rather than individual nutrients. A product labeled for bones may contain calcium, magnesium, vitamin D and vitamin K. A sleep product may contain magnesium plus botanicals. An immune product may contain zinc along with several vitamins and herbs.
A clinician who asks only, “Do you take magnesium?” may receive a no even though magnesium is present in two combination products.
This is one reason supplement interaction databases are only as useful as the ingredient list entered into them.

“Natural” Does Not Mean Pharmacologically Inactive
The word supplement covers everything from basic vitamin D to concentrated botanical extracts with meaningful pharmacologic activity.
Herbs can alter liver enzymes, intestinal transporters, platelet activity, sedation and neurotransmitter pathways. The fact that a compound comes from a plant does not prevent it from changing the concentration or effect of a prescription medication.
The National Center for Complementary and Integrative Health emphasizes this point in its review of medication and supplement interactions.
The safest response is not to assume that every herbal product is dangerous. It is to stop assuming that herbal products are automatically irrelevant to the medication list.
Starting a New Prescription Is a Good Time to Review Supplements
Medication reconciliation usually focuses on prescriptions because the immediate clinical task is to prevent prescription errors. That leaves an obvious opening for supplement interactions.
A new antibiotic is a reason to review minerals. Starting levothyroxine is a reason to review calcium, iron and multivitamins. Beginning warfarin requires a discussion about vitamin K. Starting an ACE inhibitor, ARB or spironolactone is a reason to look at potassium supplements and salt substitutes.
Medication changes can also alter whether a nutrient deserves monitoring. Long-term metformin therapy raises the B12 question. Chronic PPI use can bring B12 and magnesium into the discussion.
A five-minute supplement review at the time a prescription is started can be more useful than trying to reconstruct an interaction months later.

A good supplement plan does not begin and end with whether each ingredient is beneficial. It asks whether the supplement still makes sense in the context of everything else the person is taking.
