What Vitamins Should Men Over 50 Take? An Evidence-Based Guide

The supplement market has a fairly predictable message for men after 50. Protect the prostate. Support testosterone. Preserve memory. Strengthen the heart. Maintain energy. Keep bones strong.

Those goals are reasonable. The assumption that they require a growing collection of vitamin bottles is not.

Nutritional needs do change with age, but there is no standard supplement regimen that every man should begin at 50. Vitamin B12 absorption can become less efficient. Bone health deserves more attention. Dietary magnesium and calcium may be inadequate. Vitamin D remains important, although routine high-dose supplementation is no longer supported for otherwise healthy adults simply because they are getting older.

Some nutrients deserve less enthusiasm. Most men do not need supplemental iron unless deficiency has been demonstrated. High-dose zinc can cause copper deficiency. Selenium has not prevented prostate cancer in large randomized trials. High-dose vitamin E deserves particular caution because a major trial in men over 50 found an increased risk of prostate cancer.

There is no universal vitamin list for men over 50. A nutrient-dense diet can supply most vitamin and mineral requirements, while supplements are most useful when diet, medications, absorption or laboratory findings identify a gap.

Vitamin D deserves attention for bone and muscle health, but healthy adults ages 50 through 74 are not advised to take doses above established dietary requirements simply for disease prevention. Vitamin B12 becomes more relevant because some older adults absorb food-bound B12 less efficiently. Magnesium intake may be low when diets contain few vegetables, legumes, nuts, seeds and whole grains. Calcium requirements remain important, particularly later in life, but total dietary intake should be counted before adding a supplement.

Routine iron supplementation is usually unnecessary for adult men. Zinc is essential, but long-term high-dose zinc can create copper deficiency. Selenium and high-dose vitamin E should not be taken as prostate-cancer prevention supplements. The large SELECT trial found no prostate-cancer benefit from selenium and an increased prostate-cancer risk among men assigned 400 IU of vitamin E daily.

A basic multivitamin may be reasonable when diet is inconsistent or restricted, but it should fill nutritional gaps rather than serve as a substitute for food, exercise, sleep, metabolic health and appropriate preventive care.

Turning 50 Does Not Create a New Vitamin Requirement

A 52-year-old man who regularly eats fish, vegetables, beans, nuts, fruit, dairy or fortified alternatives and adequate protein has a different nutritional profile from a 68-year-old man taking metformin and a proton-pump inhibitor who eats few vegetables and has begun losing weight and muscle.

Age matters, but diet and health history usually tell us more about whether supplementation is needed.

Medication use becomes particularly relevant with age. Metformin can affect vitamin B12 status. Long-term acid suppression can interfere with B12 absorption from food. Diuretics can alter magnesium balance. Gastrointestinal surgery or disease can change the absorption of several nutrients.

A useful supplement plan begins with those details rather than an age printed on the front of a bottle.

Vitamin D: Important Without Becoming a Megadose

Vitamin D supports calcium absorption, bone mineralization and normal muscle function. These functions remain important for men as they age, particularly as osteoporosis, falls and loss of muscle become more clinically relevant.

The Recommended Dietary Allowance is 600 IU per day through age 70 and 800 IU per day beginning at age 71.

The Endocrine Society's 2024 guideline specifically advises against routine vitamin D supplementation above established dietary requirements in otherwise healthy adults ages 50 through 74 simply for disease prevention. It also recommends against routine vitamin D screening in that general population when there is no other clinical indication.

This does not mean vitamin D deficiency should be ignored. Osteoporosis, malabsorption, hypocalcemia, kidney disease, certain medications and other clinical circumstances can justify testing and individualized treatment.

The point is narrower: being a healthy 55-year-old man is not, by itself, an indication for 5,000 or 10,000 IU of vitamin D every day.

Vitamin B12 Becomes More Relevant With Age

Adult men need about 2.4 mcg of vitamin B12 per day. The requirement itself does not rise substantially after 50, but absorption can become less reliable.

Vitamin B12 naturally present in food must first be released from protein in the stomach. Reduced gastric acid can interfere with this process. B12 in supplements and fortified foods is already in a free form, which is why the National Institutes of Health advises adults over 50 to obtain much of their B12 from fortified foods or supplements.

Metformin and long-term acid-suppressing medications add another reason to pay attention. Vegan diets, pernicious anemia, gastric surgery, bariatric surgery, celiac disease and Crohn's disease can also increase the likelihood of inadequate B12.

Low B12 can affect red blood cell production and neurological function. Symptoms can include fatigue, numbness, tingling, balance problems and cognitive changes, but none of these symptoms is specific enough to diagnose deficiency by itself.

Magnesium: Diet First, Supplement When There Is a Reason

Men age 51 and older need approximately 420 mg of magnesium per day. Magnesium participates in hundreds of biochemical reactions involving muscle and nerve function, glucose regulation, blood pressure, energy metabolism and bone health.

Leafy vegetables, beans, lentils, nuts, seeds and whole grains are major dietary sources. Men eating these foods regularly may obtain substantial magnesium without a supplement.

Low intake becomes more likely when diets rely heavily on refined foods. Gastrointestinal disease, type 2 diabetes, long-term proton-pump inhibitor use and some diuretics can also affect magnesium status.

Magnesium supplements are often marketed for sleep, stress, blood pressure and muscle cramps. Some of those uses have supportive evidence in selected populations, but magnesium should not be treated as a universal solution for nonspecific symptoms.

Form also matters. Citrate, glycinate, oxide and L-threonate differ in absorption, gastrointestinal effects and the strength of the research supporting their more specialized claims. We review those differences in Magnesium: Which Form, What It Does, and Who Actually Needs It?.

Calcium: Men Need It Too

Bone health is sometimes treated as primarily a women's issue. Men develop osteoporosis and suffer osteoporotic fractures as well, often later in life and sometimes after years of declining bone density that went unnoticed.

Men ages 51 through 70 generally need about 1,000 mg of calcium per day. Beginning at age 71, the recommendation increases to 1,200 mg.

That is total calcium from food and supplements combined.

Dairy products, fortified plant milks, calcium-set tofu, sardines and salmon with bones and certain vegetables can make a substantial contribution. Men who already obtain enough calcium from food do not need to add the full recommended intake again as a supplement.

There is another reason not to pursue unnecessarily high calcium intakes. Research examining calcium and prostate cancer has produced inconsistent results, although some observational studies have associated high calcium intake, particularly from dairy, with greater prostate-cancer risk. This does not justify avoiding calcium or dairy. It does reinforce the value of meeting nutritional requirements without assuming that more is healthier.

Bone health also depends on resistance training, adequate protein, vitamin D, hormone status, smoking, alcohol exposure and other factors. A calcium tablet cannot substitute for those.

Iron Is Usually Not a Routine Men's Supplement

Adult men need approximately 8 mg of iron per day, and most men eating a varied diet can meet that requirement without supplemental iron.

Unlike menstruating women, men do not experience predictable monthly blood loss. New iron deficiency in an adult man therefore deserves an explanation.

Frequent blood donation can deplete iron stores. Celiac disease and gastrointestinal disorders can interfere with absorption. Ulcers, polyps, colorectal cancer and other gastrointestinal conditions can produce chronic blood loss.

An iron-containing supplement should not be used to cover unexplained deficiency without investigating why iron became low.

Iron overload is also clinically important in men. Hereditary hemochromatosis increases iron absorption and can cause progressive iron accumulation in the liver, pancreas, heart and other tissues. Men with iron overload disorders should not routinely take supplemental iron.

Zinc Is Essential, but High-Dose Zinc Is Not a Men's Health Strategy

Zinc is involved in immune function, DNA synthesis, wound healing, cell division and numerous enzymes. Adult men need about 11 mg per day.

Meat, seafood, dairy products, beans, nuts and fortified foods can provide zinc. Oysters are particularly rich sources.

Zinc is frequently added to supplements marketed for testosterone, prostate health and male vitality. The existence of zinc deficiency does not make high-dose zinc beneficial for men whose intake is already adequate.

The adult tolerable upper intake level is 40 mg per day. Doses around 50 mg or more taken for weeks can interfere with copper absorption, reduce immune function and lower HDL cholesterol. Long-term excessive zinc can produce clinically significant copper deficiency, including anemia and neurological symptoms.

A man's supplement routine should therefore be reviewed for duplicated zinc. It is common to find zinc in a multivitamin, immune formula, prostate formula and standalone product at the same time.

Selenium Is Not a Prostate-Cancer Prevention Supplement

Selenium is an essential mineral involved in thyroid function, reproduction, DNA synthesis and antioxidant enzymes. Adults need approximately 55 mcg per day, and seafood, meat, poultry, eggs, dairy products, grains and Brazil nuts can provide substantial amounts.

Selenium became especially popular with men because early observational research suggested that it might lower prostate-cancer risk.

The large Selenium and Vitamin E Cancer Prevention Trial, better known as SELECT, directly tested that hypothesis in more than 35,000 men age 50 and older. Supplementation with 200 mcg of selenium per day did not prevent prostate cancer.

Subsequent analyses raised additional concerns about supplementing men who already had adequate selenium status. There is no good evidence supporting routine selenium supplementation for prostate-cancer prevention.

This is an important example of why plausible biology and observational associations are not enough to establish that adding more of a nutrient improves health.

High-Dose Vitamin E Deserves Particular Caution in Men

Vitamin E is an essential fat-soluble antioxidant, and food sources such as nuts, seeds and vegetable oils belong in a healthy diet.

High-dose vitamin E supplements are a different matter.

SELECT randomly assigned men to vitamin E, selenium, both supplements or placebo. Men taking 400 IU of synthetic vitamin E daily ultimately had a statistically significant 17% relative increase in prostate-cancer incidence compared with men assigned placebo.

In absolute terms, the National Cancer Institute reported approximately 76 prostate cancers per 1,000 men in the vitamin E group versus 65 per 1,000 in the placebo group over about seven years.

The finding does not suggest that vitamin E naturally present in foods is dangerous. It argues against using high-dose vitamin E supplements as a prostate-health or anti-aging strategy.

Beta-Carotene Supplements Are Not a Substitute for Vegetables

Beta-carotene is a carotenoid that the body can convert to vitamin A. Foods rich in carotenoids, including carrots, squash, sweet potatoes and leafy greens, are part of a healthy dietary pattern.

High-dose beta-carotene supplements do not reproduce the effects of eating those foods.

Large randomized trials found increased lung-cancer risk among smokers and people with significant asbestos exposure who received high-dose beta-carotene. The U.S. Preventive Services Task Force recommends against beta-carotene supplementation for prevention of cardiovascular disease or cancer.

Former smokers should also be cautious about high-dose beta-carotene formulations.

Be Careful With High-Dose B-Complex Products

B vitamins are essential, but supplement labels commonly contain doses far above nutritional requirements.

Vitamin B6 is a useful example. It supports numerous metabolic and neurologic functions, yet chronic excessive supplemental B6 can itself cause peripheral neuropathy. The U.S. tolerable upper intake level is 100 mg per day for adults, while European regulators adopted a considerably lower adult limit of 12 mg per day after reviewing neuropathy data.

A high number on a label is not evidence that a product provides greater benefit. Men taking a multivitamin, B-complex, energy product and other combination supplements can accumulate surprisingly large amounts of the same B vitamins without realizing it.

What About a Daily Multivitamin?

A basic multivitamin can be reasonable when diet is inconsistent, calories are restricted or major food groups are routinely avoided. It can cover small nutritional gaps without requiring a separate supplement for every micronutrient.

The evidence does not support treating a multivitamin as comprehensive protection against cardiovascular disease or premature death.

The Physicians' Health Study II is particularly relevant because it enrolled 14,641 male physicians age 50 and older. After more than a decade, men assigned a daily multivitamin did not have fewer major cardiovascular events, heart attacks, strokes or cardiovascular deaths. The trial did report a modest 8% reduction in total cancer incidence, but there was no significant reduction in prostate cancer or cancer mortality.

More recent research in broader populations has similarly failed to show that routine multivitamin use prolongs life.

A multivitamin can still be useful. Its most defensible purpose is nutritional coverage, not the promise that one pill prevents the major diseases of aging.

Men Over 50 Should Look Beyond the Vitamin Bottle

The largest health risks facing men after 50 are rarely the result of an isolated vitamin deficiency.

Blood pressure, apoB-containing lipoproteins, glucose regulation, visceral fat, smoking, alcohol, sleep apnea, physical inactivity and loss of muscle all have substantial implications for future cardiovascular and metabolic health. Prostate health changes with age. Bone density can decline. Cognitive risk is shaped by many of the same vascular and metabolic factors that affect the heart.

Nutrition belongs in that discussion, but vitamins occupy a relatively small part of it.

A man taking an elaborate supplement regimen while ignoring hypertension, poor sleep, central obesity, low fitness or progressive muscle loss has focused on the easier part of prevention.

Protein and Resistance Training Matter More Than Another Vitamin

Loss of skeletal muscle becomes increasingly important with age. Muscle supports strength, mobility, insulin sensitivity, glucose disposal, bone loading and physical independence.

Meeting micronutrient requirements does not prevent age-related muscle loss if protein intake and resistance training are inadequate.

For healthy aging, we generally place considerably more emphasis on sufficient protein, progressive strength training and maintenance of lean mass than on adding marginal vitamins to an already adequate diet.

This is also why body composition can provide information that a bathroom scale cannot. Weight can remain stable while muscle declines and visceral fat increases.

A man taking metformin and long-term acid suppression deserves more attention to B12. Someone eating few vegetables, nuts, beans or whole grains may have a magnesium intake problem. A man avoiding dairy and fortified alternatives may need a closer look at calcium. Unexplained low iron requires investigation rather than routine replacement. A man taking multiple prostate and testosterone products may already be consuming more zinc, selenium or vitamin E than he realizes.

Laboratory testing is useful when it answers a specific question. B12 and methylmalonic acid can help when B12 deficiency is suspected. Ferritin and transferrin saturation help establish iron status. Vitamin D testing has a role in selected clinical circumstances but is not required as routine screening for every otherwise healthy man.

Men over 50 generally do not need a long age-based supplement list. Vitamin D, B12, magnesium and calcium are worth reviewing because aging, diet and medications can change their relevance. A basic multivitamin may make sense when food intake leaves predictable gaps.

Iron usually should not be taken without a reason. Zinc should not be pushed to high doses simply because it appears in men's health products. Selenium has not been shown to prevent prostate cancer, and high-dose vitamin E has produced evidence of harm in men.

The more useful question is whether a particular nutrient is inadequate, whether supplementation corrects that problem and whether anything else in the supplement regimen is duplicating the dose.

For men interested in healthy aging, the major work remains familiar: maintain muscle, stay physically active, eat well, control cardiovascular and metabolic risk, sleep adequately and use diagnostic testing where it can meaningfully change care. Supplements can support that work when they have a reason to be there.
