A central part of this opportunity is the chance to work directly with Dr. Kathryn Retzler, who founded HormoneSynergy and has spent more than 25 years caring for patients in hormone medicine, metabolic health, preventive cardiology, healthy aging, and preventive longevity medicine.
Dr. Retzler has also spent years teaching other physicians. Her educational work has included national programs on women's longevity, hormone physiology, body composition, hormone testing and monitoring, BHRT delivery, treatment complications, and complex clinical cases. One example is her Women's Longevity and Hormone Health physician education program.
The goal of mentorship at HormoneSynergy is not to have another physician memorize a set of protocols. Protocols are useful, but experienced medicine depends on knowing when they apply, when they do not, and when a patient's situation deserves a closer look.
Clinical judgment includes:
- Determining whether hormone therapy is appropriate for the individual patient
- Recognizing which laboratory findings are clinically meaningful and which may not change management
- Knowing what should be evaluated before treatment begins
- Considering symptoms, medical history, risk factors, medications, and laboratory findings together
- Recognizing when hormone treatment may help and when another problem deserves attention first
- Knowing how aggressively treatment should be adjusted and when restraint is the better choice
- Recognizing when a patient should be referred or evaluated more broadly
That kind of judgment develops over years of seeing patients, following outcomes, reviewing the literature, recognizing what worked and what did not, and learning that good medicine rarely fits perfectly into an algorithm.
Initial Clinical Focus: Hormone Medicine
The incoming physician's initial work will primarily involve hormone replacement therapy for women and men. Depending on experience and scope of practice, that may include:
- Perimenopause and menopause evaluation
- Estradiol and progesterone therapy
- Testosterone therapy for appropriate women and men
- Hormone laboratory interpretation
- Thyroid assessment and treatment within scope
- Medication monitoring and adjustment
- Risk assessment and informed decision-making
- Longitudinal follow-up
HormoneSynergy does not treat hormone therapy as a stand-alone anti-aging intervention. Hormones can be valuable medical tools, but their use requires appropriate patient selection, monitoring, and clinical restraint. Who We Are Looking For
The physician joining HormoneSynergy must be able to manage patients independently within the legal limits of the physician's scope of practice. Experience matters, particularly when it comes to prescribing, longitudinal patient care, and recognizing when a seemingly straightforward hormone complaint may require a broader medical evaluation.
Required qualifications include:
- ND, MD, or DO degree
- Current, active, unrestricted Oregon physician license
- Full prescribing authority appropriate to the position
- Current and valid DEA registration
- Ability to prescribe and manage prescription medications, including controlled medications when clinically appropriate and legally permitted
- Current professional credentials in good standing
- Professional liability insurance with minimum coverage of $1 million per occurrence and $3 million aggregate, or ability to obtain required coverage
- Ability to order and interpret appropriate laboratory and diagnostic studies
- Strong clinical documentation and communication skills
- Sound independent medical judgment
Prior experience prescribing and monitoring hormone replacement therapy is strongly preferred.
The Person Matters
The right physician for HormoneSynergy will bring more than credentials and prescribing experience. We are looking for someone who thinks carefully, communicates well, and understands the responsibility that comes with caring for patients who are paying directly for their medical care and expecting thoughtful guidance in return.
Good clinical judgment includes knowing the limits of what we know. A physician should be able to tell a patient when more information is needed, when a treatment is unlikely to help, or when an intervention being promoted elsewhere is not medically justified.
We also value professional independence. Dr. Retzler's role is to share the clinical knowledge and experience she has developed over more than two decades, not to create physicians who need her approval for every decision. The goal is to develop clinicians who understand the HormoneSynergy approach, exercise sound medical judgment, and become comfortable managing an increasingly substantial patient panel.
Temperament matters as well. Longevity medicine attracts strong opinions, enthusiastic patients, rapidly changing technologies, and a steady stream of new treatments. We are looking for someone who can remain curious without becoming credulous, disagree professionally, communicate uncertainty without losing a patient's confidence, and resist the temptation to turn every new idea into a service.

There are valuable developments taking place in hormone medicine, cardiovascular prevention, metabolic health, diagnostic imaging, body composition, cognitive assessment, and healthy aging. There is also no shortage of treatments, tests, supplements, peptides, and technologies being promoted before their clinical value is clear.
Our approach is to evaluate those tools on their merits. Some belong in patient care. Others may be promising but premature. Some add expense without adding useful information.
A good cash-pay medical practice should not depend on finding more things to sell a patient. It should depend on making good decisions about what that particular patient actually needs.
That may include hormone therapy, cardiovascular evaluation, metabolic treatment, body composition testing, resistance training, medication, nutrition, or attention to sleep and recovery. In other cases, good medical judgment may mean ordering less, prescribing less, or simply following the patient before doing anything further.
We want a physician who is comfortable practicing in that environment.
A Different Way to Practice Medicine
For many physicians, the appeal of private practice is the opportunity to spend more time with patients and exercise greater clinical independence. The difficulty is that owning a practice often leaves less time for medicine, not more. Staffing, payroll, technology, compliance, scheduling, marketing, billing, and patient acquisition quickly become part of the job.
Other physicians remain in larger medical systems because they do not want those responsibilities, even when the pace and structure of conventional practice are no longer how they would choose to work.
HormoneSynergy offers a middle ground. The physician can develop a patient panel within an established medical practice while the business infrastructure is already in place.
The position can begin on a limited schedule and expand as the physician becomes established. HormoneSynergy provides the clinic, staff support, scheduling, administrative systems, patient coordination, marketing, and existing practice visibility. Dr. Retzler provides mentorship during the transition. The incoming physician brings the clinical ability, professionalism, and commitment necessary to build long-term relationships with patients.

Production-Based Physician Compensation
The physician is employed by HormoneSynergy as a W-2 employee and paid through regular payroll. The position may begin part-time and increase in clinical availability as the patient panel grows.
The physician receives 37% of collected professional-service revenue attributable to services personally provided by the physician.
Potential production compensation may begin at approximately $55,000 for an initially limited schedule and reach approximately $185,000 at $500,000 in annual physician-service collections. The 37% production formula is uncapped. For illustration, $150,000 in annual physician-service collections would generate $55,500 in physician compensation; $250,000 would generate $92,500; $400,000 would generate $148,000; and $500,000 would generate $185,000. These examples illustrate the compensation formula and are not income guarantees.
HormoneSynergy pays applicable employer payroll taxes and provides the practice infrastructure, including patient acquisition, clinical and administrative staff, scheduling, EMR, office facilities, laboratory coordination, marketing, and operational support.
Outside professional activities may continue as long as they do not interfere with agreed-upon HormoneSynergy responsibilities, patient continuity, professional obligations, or the interests of the practice.
The physician remains responsible for maintaining all required professional licenses, registrations, DEA credentials, continuing education, and professional liability coverage. Employee payroll withholdings will be handled through regular payroll as required by law. Benefits eligibility, specific responsibilities, scheduling, and other employment terms will be established in the final written physician employment agreement and must comply with applicable Oregon and federal requirements.

What does mentorship with Dr. Retzler involve?
The physician will become familiar with the HormoneSynergy clinical model through orientation, case review, collaborative patient management, and progressively greater independence. The goal is to develop an independent HormoneSynergy physician, not to create a permanently supervised position.
Does the role extend beyond hormone therapy?
Potentially. Hormone medicine is the initial focus. As the physician develops within the practice, there may be opportunities to expand into metabolic health, preventive cardiology, body composition, bone health, cognitive health, weight management, and other areas of preventive longevity medicine.
