What Does Five Times More Akkermansia Mean?
The label difference is straightforward. Akkermansia 500 PRO contains five times as many measured viable organisms as standard Pendulum Akkermansia.
The biological difference is harder to quantify.
Probiotics do not behave like conventional medications, where increasing a dose can sometimes produce a predictable increase in blood concentration or pharmacologic effect. Whether a supplemented organism survives, persists and meaningfully changes the intestinal environment depends on the organism, the existing microbiome, diet, fermentable substrates and characteristics of the person taking it.
Pendulum has described lower-dose Akkermansia as appropriate for maintenance and the 500-million-AFU formula as a higher-potency replenishment option. That is a reasonable way to understand the product lineup, but it should not be interpreted as a clinically established threshold. Human outcome trials have not demonstrated that 500 million AFU is consistently superior to 100 million AFU.
What the Human Research Tells Us
Akkermansia muciniphila has moved further into human research than many organisms promoted in the probiotic marketplace. The findings are promising, but they also show why product claims need to remain measured.
A 2019 randomized proof-of-concept trial studied adults with overweight or obesity and insulin resistance. Participants received 10 billion organisms per day in either live or pasteurized form. The more notable metabolic findings occurred with the pasteurized preparation. The dose was also substantially higher than either of the Pendulum products discussed here, and the experimental preparations were not the finished Pendulum formulas.
A 2025 randomized trial involving adults with overweight or obesity and type 2 diabetes added another important piece of information. Across the full study population, supplementation did not significantly outperform placebo for body weight or HbA1c. Participants who began with relatively low intestinal levels of Akkermansia, however, appeared to colonize more successfully and showed greater metabolic improvement. Those who already had relatively high levels showed much less response.
Two randomized studies published in 2026 evaluated pasteurized Akkermansia rather than Pendulum's live strain. One reported less weight regain following diet-induced weight loss. Another trial involving metabolic syndrome did not meet its primary insulin-sensitivity endpoint across the full study population, although exploratory analyses again suggested that baseline Akkermansia abundance may influence response.
The emerging picture is more nuanced than simply assuming that more Akkermansia is better. The starting microbiome may be an important part of the equation.
When Standard Akkermansia May Be Enough
The 100-million-AFU formula provides the same Pendulum strain found in 500 PRO and the same amount of chicory inulin. For someone looking for straightforward, targeted Akkermansia support, there may be no reason to assume that the higher-dose formula is automatically necessary.*
Standard Akkermansia may also provide a reasonable starting point when introducing a targeted organism before deciding whether a more concentrated formulation has a useful role. That is different from treating 500 PRO as an inevitable upgrade.
Where Akkermansia 500 PRO Fits
Akkermansia 500 PRO is the more concentrated option. Each capsule provides 500 million AFU of the same WB-STR-0001 strain while leaving the amount of chicory inulin unchanged.
That makes it a practical choice when a higher-dose Akkermansia approach has been intentionally selected.* The formulation increases one specific organism without introducing an entirely different group of probiotic species or requiring several overlapping products.
What cannot currently be claimed is that 500 PRO will work five times better, colonize five times more effectively or produce five times greater metabolic improvement. There is no direct clinical evidence supporting that interpretation.

The Inulin Dose Does Not Change
Both formulas contain 276 mg of chicory inulin. This is worth noting for people who are sensitive to fermentable fibers.
Moving from standard Akkermansia to 500 PRO increases the Akkermansia dose without increasing the labeled amount of inulin. Moving in the opposite direction does not meaningfully reduce exposure to the prebiotic ingredient. Someone who develops bloating or other digestive symptoms associated with inulin should not assume that switching between these two formulas will necessarily resolve the problem.
Should You Take Both?
For most purposes, using the two products together would simply duplicate the same strain and the same prebiotic ingredient. One capsule of each would provide 600 million AFU of WB-STR-0001 along with twice the amount of chicory inulin.
There is no established clinical advantage to creating that particular dose. A deliberate microbiome strategy generally makes more sense than accumulating overlapping probiotic products.
Akkermansia Is Only One Part of the Gut Ecosystem
The attention surrounding Akkermansia muciniphila is supported by interesting biology and a growing human research base. It should not turn a single organism into a substitute for broader microbiome health.
Dietary fiber, polyphenol-rich plant foods, physical activity, metabolic health, medications, sleep and the surrounding microbial community all influence the intestinal environment. Supplementing one organism cannot compensate for a diet and lifestyle that do not support the broader ecosystem.
