Diamine oxidase, or DAO, is one of the enzymes involved in histamine metabolism and is particularly important at the intestinal barrier. When histamine enters the digestive tract through food, DAO helps break it down before much of it reaches the circulation.

This provides a reasonable biological basis for using supplemental DAO shortly before meals in people who repeatedly react to histamine-rich foods. The clinical evidence, however, is not strong enough to turn every headache, episode of bloating or glass of red wine that “doesn't agree with me” into a diagnosis of histamine intolerance. Symptoms overlap with food allergy, gastrointestinal disorders, migraine, medication effects, mast-cell disorders and many other conditions. Serum DAO testing can add information in selected cases, but it cannot establish the diagnosis by itself.

DAO supplementation therefore makes the most sense as a focused experiment when the food relationship is convincing. Histamine Is Not Something the Body Is Trying to Get Rid Of

Histamine is formed from the amino acid histidine and communicates through several receptor systems, generally described as H1, H2, H3 and H4 receptors. Those receptors are distributed differently throughout the body, which helps explain why histamine can influence such different functions.

It participates in the familiar redness, swelling and itching of an allergic response, but its biology extends well beyond allergy. Histamine influences stomach-acid secretion, blood vessels, smooth muscle, sensory nerves, wakefulness and communication within the immune system.

This is why the phrase “too much histamine” can become misleading. A person can have excessive histamine signaling in one setting, impaired metabolism of histamine coming from food in another, or symptoms that resemble a histamine reaction without histamine being the primary problem at all.

Food Adds Another Source of Histamine

Histamine is also present in food. Bacteria can convert histidine into histamine, so concentrations tend to rise in foods that are aged, fermented, cured or stored under conditions that permit microbial activity. Mature cheeses, cured meats, fermented foods, wine, beer and certain fish are familiar examples. Leftovers can also accumulate more biogenic amines with storage, although actual concentrations vary considerably from one food and preparation method to another.

This variability is one reason rigid online lists of “high-histamine foods” are less precise than they appear. A food may contain a relatively modest amount in one setting and considerably more in another. Individual tolerance also varies, and a meal contains many compounds besides histamine that can influence how someone feels afterward.

Where Diamine Oxidase Fits

Diamine oxidase is highly expressed in the intestinal mucosa, particularly in the small intestine. Within this environment, it contributes to the metabolism of extracellular histamine before dietary histamine can move from the intestinal lumen into the circulation.

The body has another important histamine-metabolizing enzyme called histamine N-methyltransferase, or HNMT. The two enzymes are not interchangeable. HNMT works intracellularly in a number of tissues, while intestinal DAO is especially relevant to histamine encountered through food.

That physiology explains why an oral DAO supplement is generally taken shortly before eating. The purpose is to have additional enzymatic activity present around the time dietary histamine reaches the gastrointestinal tract.

It also explains what DAO supplementation is not expected to do. It is not a systemic antihistamine. It does not shut down histamine production by mast cells, and it does not block H1 or H2 receptors throughout the body.

Histamine Intolerance Is Harder to Diagnose Than the Internet Suggests

The term histamine intolerance is generally used to describe symptoms attributed to an imbalance between the amount of histamine entering through the diet and the body's ability to metabolize that histamine. Reduced intestinal DAO activity is one proposed mechanism.

The difficulty is that there is no symptom pattern unique to histamine intolerance. Gastrointestinal discomfort, diarrhea, abdominal distention, headache, flushing, nasal symptoms, itching and palpitations can occur for many reasons. A person can collect half a dozen of these symptoms and still not have impaired dietary histamine metabolism.

Research using blinded histamine challenges illustrates the problem. In one placebo-controlled investigation of people who believed they had histamine intolerance, most participants could not have the diagnosis confirmed. Placebo reactions were common, while serum DAO values did not cleanly separate those who reacted from those who did not.

This does not mean dietary histamine reactions are imaginary. It means symptoms alone are not sufficiently specific to identify their cause.

A DAO Blood Test Is Not a Verdict

Serum DAO measurement is sometimes promoted as though a low value proves histamine intolerance. The evidence does not support using it that way.

A 2023 study comparing people with clinically suspected histamine intolerance and healthy controls found lower DAO concentrations among those with a higher clinical probability of the condition. There was still substantial overlap between groups, and the investigators concluded that serum DAO should be interpreted as an additional piece of information rather than used as a stand-alone diagnostic test.

A large population study reached a similar practical concern. Many people without meaningful reactions to dietary histamine had DAO values below previously proposed “normal” thresholds. A laboratory number therefore needs clinical context.

What Can Affect DAO Activity?

DAO activity does not exist in isolation. Intestinal mucosal health appears to matter because the enzyme is produced in the gut lining. Genetic variation in the AOC1 gene, which encodes DAO, may also influence enzyme activity. Medications, gastrointestinal disease, diet and other environmental factors have been discussed as possible contributors.

This broader context is one reason we are cautious about making “DAO deficiency” a permanent identity. A patient with a new pattern of food reactions deserves more than a supplement recommendation. The clinical history should still ask when symptoms began, which foods reliably provoke them, whether alcohol is involved, whether there has been a change in gastrointestinal health, which medications are being used and whether another diagnosis fits better.

What the Research on Supplemental DAO Actually Shows

The evidence for oral DAO supplementation is encouraging enough to justify clinical interest, but it remains modest.

One frequently cited study followed 28 adults identified as having histamine intolerance. Participants took DAO before meals for four weeks. Reported symptoms improved during supplementation and increased again after DAO was withdrawn. The finding is interesting, particularly because the symptoms moved in both directions with treatment and withdrawal, but the study was open-label and did not include a placebo-treated control group.

A separate randomized, double-blind study evaluated DAO supplementation in 100 people with episodic migraine and documented DAO deficiency. The DAO-treated group experienced a reduction in headache duration compared with placebo. That does not establish DAO as a general migraine treatment, but it adds evidence that impaired histamine degradation may be clinically relevant in a selected population.

Recent reviews continue to describe a possible benefit from DAO supplementation while emphasizing the same limitations: small studies, inconsistent diagnostic criteria, short treatment periods and the lack of standardized methods for identifying histamine intolerance. A larger randomized, placebo-controlled trial of dietary intervention and DAO supplementation has been designed specifically because better evidence is still needed.

There is another detail worth keeping in view when reading this literature. Some reviews of dietary histamine management and DAO supplementation have financial ties to manufacturers of DAO ingredients. That does not make the findings invalid, but disclosure matters when the clinical literature is still relatively small.

A Low-Histamine Diet Should Be an Investigation, Not a Permanent Food Philosophy

When dietary histamine genuinely appears to be involved, a temporary reduction in higher-histamine foods followed by thoughtful reintroduction can be more informative than progressively eliminating foods forever.

The objective is to find out whether there is a reproducible relationship between exposure and symptoms. If symptoms improve during restriction and return with reintroduction, the pattern becomes more clinically useful. If nothing changes, there is little reason to keep narrowing the diet simply because a food appeared on an online histamine list.

Long-term restrictive eating can reduce dietary variety and make social eating unnecessarily difficult. It can also distract from a gastrointestinal condition, food allergy, migraine disorder, medication effect or another medical explanation that deserves attention.

Histamine Intolerance Is Not the Same as Food Allergy

An IgE-mediated food allergy involves immune recognition of a specific allergen and can progress to anaphylaxis. DAO supplementation is not a treatment or preventive strategy for that process.

Anyone experiencing throat tightness, tongue or facial swelling, difficulty breathing, wheezing, faintness or a previous anaphylactic reaction needs appropriate allergy and medical evaluation. Taking an enzyme before eating the responsible food is not an acceptable substitute.

DAO and Mast Cell Activation Are Also Different Questions

Mast cells can release histamine that is produced inside the body. DAO supplementation, by contrast, is aimed primarily at histamine present within the digestive tract. Someone with a true mast-cell disorder may therefore have histamine-related symptoms for reasons that supplemental intestinal DAO cannot reasonably be expected to correct.

There can be overlap in symptoms, which is precisely why assigning every flushing episode, headache or food reaction to “histamine intolerance” is clinically unsatisfying.

When a Trial of DAO May Be Reasonable

A short trial is most logical when symptoms have a convincing relationship to meals, particularly meals known to contain more histamine, and when the goal is to test whether greater intestinal histamine degradation changes that response. Taking DAO before the relevant meal fits the proposed mechanism much better than taking it randomly during the day.

Improvement can be useful information, but response to a supplement still does not prove a diagnosis. Symptoms fluctuate naturally, diet changes at the same time, and expectation can influence symptom reporting. If a person continues to require progressively broader food avoidance or symptoms are becoming more severe, the clinical evaluation deserves more attention than the supplement regimen.

DAO has one of the more straightforward physiological rationales among supplements used for food-related symptoms. An enzyme is present at the intestinal barrier, food can deliver a meaningful histamine load, and supplemental enzyme activity may increase the amount of histamine metabolized before absorption.

What remains less straightforward is deciding who actually has clinically important impairment of that pathway.

For that reason, we prefer to start with the pattern rather than the label. Does the reaction consistently follow the same types of foods? Does freshness or fermentation change the response? What happens with a temporary dietary modification followed by reintroduction? Are there gastrointestinal symptoms suggesting something else is happening in the intestinal mucosa? Could medications, alcohol, migraine, allergy or another condition explain the presentation?

DAO supplementation may fit into that process. It should not replace it.
