A three-year randomized, double-blind, placebo-controlled trial evaluated 2,000 FU of nattokinase daily in 265 healthy adults without clinical cardiovascular disease. The study did not find significant improvement in carotid intima-media thickness, arterial stiffness, blood pressure, laboratory risk markers, or cognitive decline.
This result provides useful context. The participants were generally healthy and at relatively low cardiovascular risk, so the findings may not answer every question about nattokinase in populations with different risk profiles. It does show that measurable fibrinolytic activity should not automatically be interpreted as proven plaque regression or protection from cardiovascular events.
Current evidence supports describing nattokinase as a promising circulation and fibrinolytic-support ingredient with developing clinical research.* Larger trials are still needed to determine whether its biological effects translate into fewer heart attacks, strokes, blood clots, or other long-term outcomes.
