What Is NAC?
N-acetyl-L-cysteine, usually shortened to NAC, is an acetylated form of the amino acid L-cysteine.
Its importance comes largely from what happens after it enters the body. NAC can provide cysteine for the synthesis of glutathione, a tripeptide made from cysteine, glutamate and glycine.
Cysteine availability can be an important limiting factor in glutathione synthesis. That is one reason NAC has been studied so extensively in redox biology.
NAC also has sulfur-containing thiol chemistry of its own, and modern research suggests that its biological effects are more complicated than simply labeling it a direct “free-radical scavenger.”

Glutathione is sometimes called the body's “master antioxidant.” We understand why the phrase became popular, but it can make the biology sound simpler than it is.
Glutathione is better described as one of the body's major intracellular antioxidant and redox-regulating systems. It participates in the reduction of peroxides, recycling of other antioxidant systems, conjugation of certain compounds and maintenance of cellular redox balance.
It works alongside catalase, superoxide dismutase, glutathione peroxidase, thioredoxin, Nrf2-related pathways and many other systems.

NAC and Detoxification: What We Actually Mean
Few words in the supplement industry are abused more than detox.
Your liver is not a dirty filter that needs to be flushed. Detoxification is a continuous set of biochemical processes involving the liver, kidneys, gut, bile, lungs and other tissues.
Glutathione participates in this physiology, including conjugation reactions involved in processing certain endogenous and exogenous compounds. Because NAC can provide cysteine for glutathione synthesis, it may support these normal pathways.*

Does NAC Work as an Antioxidant?
Yes, but the mechanism is more interesting than most supplement descriptions suggest.
NAC contains a thiol group and can participate directly in certain reduction reactions. However, reviews of NAC pharmacology have pointed out that it should not simply be thought of as a powerful antioxidant molecule that directly neutralizes every reactive oxygen species it encounters.
A major part of its value is its ability to provide cysteine for glutathione synthesis, particularly when cysteine or glutathione availability is limited.
This is another reason we prefer the language “supports antioxidant defense” rather than claiming that NAC broadly “protects the body from free radicals.”
NAC Has Real Pharmacology
NAC is unusual among compounds sold as dietary supplements because acetylcysteine also has a long history of medical use.
In medical settings, acetylcysteine is used as a mucolytic and is an established treatment for acetaminophen toxicity, where restoration of hepatic glutathione is clinically important.
That history demonstrates that NAC is biologically active. It does not mean an over-the-counter NAC capsule should be used to treat an overdose, respiratory disease, liver disease or another medical condition.
Those are medical uses involving specific formulations, doses, routes and clinical supervision.
NAC or Glutathione?
They are related, but they are not the same thing.
NAC provides a precursor. The body converts it into cysteine that can be used, along with glutamate and glycine, to synthesize glutathione.
Glutathione supplements provide glutathione itself. Formulation then becomes important because oral glutathione products differ in delivery technology, stability and supporting evidence.
There is no reason to pretend that one approach is universally better.

Why Does NAC Smell Like Sulfur?
If you open a bottle of NAC and notice a sulfur-like smell, you are not imagining it.
NAC is a sulfur-containing compound, and the characteristic odor may be described as sulfurous or somewhat similar to cooked eggs.
This can be a normal characteristic of NAC and does not by itself mean that the product has spoiled.
